In order to provide a more personalized primary care experience, Dr. Zapata is changing to a concierge, membership-based model beginning December 14, 2026. His practice size will be intentionally limited to devote additional time and availability to patients.

What Will Be the Same

You'll receive attentive care from Dr. Zapata, who already knows you and your medical history.
Our offices will remain at 12797 Forest Hill Blvd, Suite B Wellington, FL 33414 and 6334 Forest Hill Blvd, Suite # 6334 Greenacres, FL 33415.
Medical, covered services will still be billed to insurance. Your annual membership fee is separate and is paid for out of pocket.

What Will Change

Longer visits. Appointments will be scheduled for 30-60 minutes, ensuring time for all your questions to be answered and concerns to be addressed.
A direct line to Dr. Zapata. Reach Dr. Zapata directly, even outside of business hours, for urgent needs. Oftentimes a conversation with the doctor who knows you can save a trip to urgent care or the ER.
An intentionally smaller practice. Starting this December, Dr. Zapata will only see patients enrolled in the concierge membership. Those not interested in concierge care, are welcome to stay with the practice and receive care from the Nurse Practitioners on staff.

Couples rate is a 10% discount.

Your first installment will be charged upon enrollment and will apply to the membership period beginning December 2026.
